ABSTRACT

Objective: To evaluate the efficacy of endoscopic transnasal surgery for sinonasal and skull base adenoid cystic carcinoma (ACC), and to analyze the prognostic factors. Methods: Data of 82 patients (43 females and 39 males, at a median age of 49 years old) with sinonasal and skull base ACC who were admitted to XuanWu Hospital, Capital Medical University between June 2007 and June 2021 were analyzed retrospectively. The patients were staged according to American Joint Committee on Cancer (AJCC) 8th edition. The disease overall survival(OS) and disease-free survival(DFS) rates were calculated by Kaplan-Meier analysis. Cox regression model was used for multivariate prognostic analysis. Results: There were 4 patients with stage Ⅱ, 14 patients with stage Ⅲ, and 64 patients with stage Ⅳ. The treatment strategies included purely endoscopic surgery (n=42), endoscopic surgery plus radiotherapy (n=32) and endoscopic surgery plus radiochemotherapy (n=8). Followed up for 8 to 177 months, the 5-year OS and DFS rates was 63.0% and 51.6%, respectively. The 10-year OS and DFS rates was 51.2% and 31.8%, respectively. The multivariate Cox regression analysis showed that late T stage and internal carotid artery (ICA) involvement were the independent prognostic factors for survival in sinonasal and skull base ACC (all P<0.05). The OS of patients who received surgery or surgery plus radiotherapy was significantly higher than that of patients who received surgery plus radiochemotherapy (all P<0.05). Conclusions: Endoscopic transonasal surgery or combing with radiotherapy is an effective procedure for the treatment of sinonasal and skull base ACC. Late T stage and ICA involvement indicate poor prognosis.

ABSTRACT

Objective: To examine the safety and effectiveness of laparoscopic individualized surgical treatment for chronic traumatic diaphragmatic hernia (CTDH). Methods: The clinical data and follow-up data of 29 CTDH cases admitted to the Qilu Hospital of Shandong University or the First Affiliated Hospital of Shandong First Medical University from June 2015 to January 2023 were retrospectively analyzed. There were 21 males and 8 females, aged (49.4±17.8) years (range: 19 to 79 years). The main clinical manifestations were symptoms of the digestive system and respiratory system, and only 4 cases were asymptomatic. All patients received laparoscopic treatment (conversion to open surgery was not excluded). Intraoperative exploration (location of the hernia, contents of the hernia, diameter of the hernia ring), surgical conditions (surgical repair plan, operation time, blood loss, postoperative complications) and postoperative follow-up were analyzed and discussed. Results: Laparoscopic repair was successfully completed in 27 cases, including simple suture in 6 cases, suture and patch repair in 17 cases, the anterior abdominal wall muscle flap reversal suture and patch repair of in 3 cases, and patch bridge repair in 1 case. The operation time was (112.8±44.7) minutes (range: 60 to 200 minutes). The amount of bleeding (M(IQR)) was 35 (58) ml (range: 10 to 300 ml). The other 2 patients were converted to laparotomy. Except for one patient with transverse colon strangulation necrosis who died of aggravated pulmonary infection after surgery, the remaining 28 patients were discharged successfully. The follow-up time was 36 (24) months (range: 1 to 60 months). During the follow-up period, only two patients had occasional left upper abdominal discomfort. Twenty-seven patients with left diaphragmatic hernia had no recurrence, and the symptoms of 1 patient with right diaphragmatic hernia were relieved. Conclusion: Customized laparoscopic surgical repair for CTDH according to the location and size of the diaphragmatic defect can achieve good surgical results.

ABSTRACT

Background@#and Purpose Orthostatic hypotension (OH) is common in patients with Parkinson’s disease (PD). Early recognition OH is required with sensitive assessments. The purpose of this study was to determine whether blood pressure (BP) changes during exercise can predict the occurrence of OH in PD. @*Methods@#This prospective cohort study included 80 consecutive patients with PD. All patients agreed to participate in a baseline evaluation and cardiopulmonary exercise test (CPET).According to the initial active standing test (AST), those without OH (PD-nonOH) at baseline had their AST results followed up for 6 months. The main outcome was defined as whether patients without OH at baseline would develop OH after 6 months. Logistic regression analysis was applied to identify the relevant variables. A nomogram was constructed based on clinical features and identified variables. The concordance index (C-index) and area under the receiver operating characteristic curve (AUC) were used to evaluate the accuracy and predictive ability of the nomogram, respectively. @*Results@#CPET results indicated that peak load, peak heart rate, heart rate recovery at 1 min, and systolic BP change (ΔSBP) were lower in those with OH than in the PD-nonOH group (p<0.05) at baseline. Logistic regression analysis indicated that peak load and ΔSBP during CPET had significant effects on OH (p<0.05). Age, sex, peak load, and ΔSBP were used to construct the nomogram model (C-index=0.761). The prediction model had an AUC of 0.782 (95% confidence interval=0.649–0.889) and a specificity and sensitivity of 70.0% and 81.8%, respectively. @*Conclusions@#This study has identified predictive factors for OH development in patients with PD. CPET could be used as a complementary examination to identify patients at a high risk of OH.

ABSTRACT

OBJECTIVES@#To study the association of ventricular septal defect (VSD) with rare variations in the promoter region of HAND2 gene, as well as related molecular mechanisms.@*METHODS@#Blood samples were collected from 349 children with VSD and 345 healthy controls. The target fragments were amplified by polymerase chain reaction and sequenced to identify the rare variation sites in the promoter region of the HAND2 gene. Dual-luciferase reporter assay was used to perform a functional analysis of the variation sites. Electrophoretic mobility shift assay (EMSA) was used to investigate related molecular mechanisms. TRANSFAC and JASPAR databases were used to predict transcription factors.@*RESULTS@#Sequencing revealed that three variation sites (g.173530852A>G, g.173531173A>G, and g.173531213C>G) were only observed in the promoter region of the HAND2 gene in 10 children with VSD, among whom 4 children had only one variation site. The dual-luciferase reporter assay revealed that g.173531213C>G reduced the transcriptional activity of the HAND2 gene promoter. EMSA and transcription factor prediction revealed that g.173531213C>G created a binding site for transcription factor.@*CONCLUSIONS@#The rare variation, g.173531213C>G, in the promoter region of the HAND2 gene participates in the development and progression of VSD possibly by affecting the binding of transcription factors.

ABSTRACT

Objective To investigate a convenient and quantitative solution to activation levels and functional characterization of CAR-T cells by inserting T cell activity-responsive promoter (TARP) nanoluciferase reporter gene system into a lentiviral plasmid containing the gene encoding the chimeric antigen receptor (CAR). Methods The recombinant plasmid was constructed by using whole gene synthesis and molecular cloning techniques. The lentivirus was packaged and was infected with human primary T lymphocytes. Flow cytometry was used to detected the positive rate of lentivirus-infected T cells. The functional characterization of CAR-T cells was identified by luciferase reporter gene system, Western blot, flow cytometry, and small animal live imaging techniques. Results The results of enzyme digestion identification and the plasmid sequencing showed that the recombinant plasmids were constructed, and flow cytometry displayed the normal preparation of CAR-T cells. This system could dynamically respond to the activation of CAR-T cells by luciferase reporter gene system. The functional assay in vitro confirmed that the system could reflect the exhaustion of CAR-T cells, and the small animal live imaging results demonstrated that the system can be used as a tracer of CAR-T cells in mice. Conclusion TARP nanoluciferase reporter gene system provides a more convenient, sensitive and quantitative method for evaluating CAR-T cells activation level, exhaustion phenotype and tracing.

ABSTRACT

OBJECTIVE To provide reference for the safety of drug use in children and the rational drug use management strategies by health administration departments. METHODS A self-designed questionnaire was used to investigate the cognition of antibiotics and the parents ’behavior of children ’s use of antibiotics ,using online and offline questionnaires. Univariate analysis was carried out on the scores of respondents ’cognition,and multivariate Logistic regression analysis was carried out on the factors affecting parents ’behavior of children ’s use of antibiotics. RESULTS A total of 1 068 questionnaires were distributed ,and 993 valid questionnaires were recovered ,with the effective recovery rate of 93.0%. Among the respondents ,43.2% of the parents ’ behavior of children ’s use of antibiotics in the past 6 months. The score of respondents ’cognition,high treatment cost and cumbersome treatment procedures ,easy access to antibiotics ,certain medication experience ,the recommendation of pharmacists in drug store ,the recommendation of relatives and friends and the publicity of mass media were positively correlated with parents ’ behavior of children ’s use of antibiotics (P<0.05). CONCLUSIONS The rate of the parents ’in Sichuan-Chongqing area is high , which is the result of a combination of several factors. Reducing the risk of behavior of children ’s use of antibiotics requires the joint efforts of the government ,medical institutions and the public. The parents ’cognition towards antibiotics should be improved ,and the acquisition of antibiotics should be standardized so as to reduce unreasonable use of antibiotics.

ABSTRACT

OBJECTIVE To study the regulatory mechanism of ethanol extract of Turkish Galls on proliferation and migration of colorectal cancer cells HCT- 116 and Caco- 2 based on janus kinase 2(JAK2)/signal transducer and activator of transcription 3 (STAT3)signaling pathway. METHODS CCK-8 method was used to detect the effects of 0.05,0.1,0.2,0.3,0.4 and 0.5 mg/mL ethanol extract of Turkish Galls on the proliferation of HCT- 116 and Caco- 2 cells after treated for 12,24,48 and 72 h. After treated with 0.1,0.3,0.5 mg/mL ethanol extract of Turkish Galls for 24 h,the migrations of HCT- 116 and Caco- 2 cells were detected by scratch test ;the level of reactive oxygen species (ROS)was detected by fluorescent probe method. The levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in cell supernatant were detected by ELISA . The phosphorylations of JAK2 and STAT 3 as well as the expressions of B-cell lymphoma- 2(Bcl-2)and Bcl- 2 associated protein X (Bax)were detected by Western blot assay. RESULTS Compared with blank control ,0.05,0.1,0.2,0.3,0.4 and 0.5 mg/mL ethanol extract of Turkish Galls could significantly inhibit cell proliferation after treated for 12,24,48,72 h(P<0.05). After treated with 0.1,0.3 and 0.5 mg/mL ethanol extract of Turkish Galls for 24 h,the scratch healing rate of 2 kinds of cells ,the levels of IL- 6 and TNF-α in the cell supernatant ,the phosphorylation of JAK 2 and STAT 3 as well as the expression of Bcl- 2 protein were all significantly decreased (P<0.05);the level of ROS and protein expression of Bax were increased significantly (P<0.05). CONCLUSIONS The ethanol extract of Turkish Galls can inhibit the proliferation and migration of HCT- 116 and Caco- 2 cells. The mechanism may be related with down-regulation of protein expression of Bcl- 2 and up-regulation of protein expression of Bax by increasing the accumulation of intracellular ROS ,down-regulating the expressions of inflammatory factors IL- 6 and TNF-α and the phosphorylation of JAK2 and STAT 3 in JAK 2/STAT3 signaling pathway.

ABSTRACT

ObjectiveTo evaluate the clinical efficacy of TDP (specific electromagnetic wave) combined with Osteoking in the treatment of knee osteoarthritis of Qi stagnation and blood stasis type. MethodA total of 104 patients with knee osteoarthritis of Qi stagnation and blood stasis type, who received conservative therapy in The Third People's Hospital of Xinjiang Uygur Autonomous Region from July 2019 to December 2021, were randomized into the control group and study group with the random number table method, 52 cases in either group. The control group was treated with TDP, and the study group with TDP and Osteoking. The treatment lasted 1 week for both groups, with 1-month follow-up. Subjective indexes of visual analog scale (VAS) score and Western Ontario McMaster Universities Osteoarthritis Index (WOMAC) score, and objective indexes of visual tenderness index and visual knee range of motion were determined before and after treatment to evaluate the pain and functions of patients. Traditional Chinese medicine (TCM) syndrome score was calculated. The serum erythrocyte sedimentation rate (ESR) and high sensitivity C-reactive protein (hs-CRP) were detected before and after treatment, and the total clinical effective rate was calculated. ResultBefore treatment, the baseline information and all the scores of the two groups were comparable. After treatment, the VAS score, WOMAC score, tenderness index, knee range of motion, and TCM syndrome score were improved in both groups (P<0.01). After the treatment, the VAS score and WOMAC score of the study group were lower than those of the control group (P<0.01) and the improvement of tenderness index in the study group was better than that in the control group (P<0.05). The knee range of motion in the study group was better than that in the control group (P=0.061). The TCM syndrome score of study group was lower than that of control group (P<0.01) after treatment. The post-treatment serum ESR and hs-CRP level in the two groups decreased significantly after treatment, and the study group were lower than those of the control group (P<0.01). The total clinical effective rate of the study group was 90.4%(47/52), as compared with the 53.8%(28/52) in the control group (P<0.05). No obvious adverse events occurred during treatment in both groups. ConclusionThe clinical efficacy of TDP combined with Osteoking in the treatment of knee osteoarthritis of Qi stagnation and blood stasis type is remarkable, which can improve knee pain and functions, alleviate TCM syndrome, and reduce inflammatory indexes, with high safety.

ABSTRACT

OBJECTIVE@#This study evaluated the effectiveness of acupuncture treatment on postoperative pain in patients with degenerative lumbar spine disease, and explored the relationship between the postoperative analgesic effect of acupuncture and the sensation of acupuncture experienced by the patients.@*METHODS@#This retrospective study analyzed the medical records of 97 patients who had undergone an operation by the same surgeon due to degenerative lumbar disease. These patients were divided into acupuncture group (n = 32), patient-controlled analgesia (PCA) group (n = 27), and oral analgesia group (n = 38) according to the different postoperative analgesic methods. During their hospitalization, patients completed daily evaluations of their pain using a visual analogue scale (VAS), and injection times of supplemental meperidine were recorded. Also, the Chinese version of the Massachusetts General Hospital Acupuncture Sensation Scale (C-MASS) was used in the acupuncture group.@*RESULTS@#Each of the three treatment groups showed significant reductions in postoperative pain, as shown by reduced VAS scores. The acupuncture group, however, had less rebound pain (P < 0.05) than the other two groups. Both the acupuncture and PCA groups experienced acute analgesic effects that were superior to those in the oral analgesia group. In addition, the higher the C-MASS index on the second day after surgery, the lower the VAS score on the fourth day after surgery. There was also a significant difference in the "dull pain" in the acupuncture sensation.@*CONCLUSION@#The results demonstrated that acupuncture was beneficial for postoperative pain and discomfort after simple surgery for degenerative spinal disease. It is worth noting that there was a disproportionate relevance between the patient's acupuncture sensation and the improvement of pain VAS score.

ABSTRACT

Objective:To investigate whether there is inflammatory reaction and cell pyroptosis in impaired glucose tolerance (IGT) rats induced by high-fat diet and the intervention effect of Huanglian Wendantang. Method:Healthy male SD rats were fed with 45% fat content diet for 20 weeks to replicate the IGT model. The rats in line with the model establish criteria were randomly divided into 3 groups, with 10 rats in each group, another 10 rats were selected as the blank control group. Huanglian Wendantang group was given 7.8 g·kg<sup>-1</sup>·d<sup>-1</sup> compound decoction of Huanglian Wendantang, and the positive control group was given 0.05 g·kg<sup>-1</sup>·d<sup>-1</sup> aqueous solution of metformin hydrochloride, with the dose volume of 10 mL·kg<sup>-1</sup>·d<sup>-1</sup>. The blank group and the model group were given the same volume of distilled water. After continuous intragastric administration for 4 weeks, the body weight, body length and abdominal circumferences were measured, the Lee's obesity index was calculated, and the levels of fasting plasma glucose (FPG) and 2-hour plasma glucose (2 h PG) were detected in each group. Serum interleukin-6 (IL-6) content was detected by enzyme-linked immunosorbent assay (ELISA). The mRNA and protein expressions of nuclear factor kappa B (NF-<italic>κ</italic>B), cysteinyl aspartate specific proteinase-1 (Caspase-1) and NOD-like receptor protein 3 (NLRP3) in liver tissues were detected by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) and Western blot. Immunofluorescence was used to detect the expression of Caspase-1 and NLRP3 in rat liver. Hematoxylin and eosin (HE) staining was used to observe the morphology of liver tissue. Result:Compared with blank group, the body weight, abdominal circumference, body length and Lee's index in model group were significantly increased (<italic>P</italic><0.05, <italic>P</italic><0.01), the levels of FPG and 2 h PG were significantly increased (<italic>P</italic><0.05, <italic>P</italic><0.01), serum IL-6 content and NF-<italic>κ</italic>B, Caspase-1 and NLRP3 gene and protein expressions in liver tissue were significantly increased (<italic>P</italic><0.01), immunofluorescence technique showed that Caspase-1 and NLRP3 expressions increased obviously in IGT rat model’s liver tissues, and inflammatory cells infiltration was observed obviously in HE stained rat liver tissue model. Compared with model group, Huanglian Wendantang and metformin hydrochloride groups could effectively reduce the body weight of IGT rats (<italic>P</italic><0.01) and abdominal obesity, and correct the levels of FPG and 2 h PG (<italic>P</italic><0.01), while effectively reducing serum IL-6 content and gene and protein expressions of NF-<italic>κ</italic>B, Caspase-1 and NLRP3 in liver tissues of IGT rats, and alleviating inflammatory cell infiltration. Conclusion:Inflammatory response exists in IGT model rats induced by high-fat diet. Huanglian Wendantang can obviously reduce its inflammatory response and alleviate liver cell pyroptosis.

ABSTRACT

Objective:To explore the effects of Huanglian Wendantang (HLWDT) on pyroptosis of skeletal muscle in rats with impaired glucose tolerance (IGT) and to explain the mechanism based on NOD-like receptor protein 3 (NLRP3)/cysteine aspartate-specific protease-1 (Caspase-1)/gasdermin D (GSDMD)/interleukin-1<italic>β </italic>(IL-1<italic>β</italic>)/IL-18 signaling pathway. Method:The SD male rats were fed with 45% high-fat diet for 20 weeks to induce the IGT model. After modeling, the rats were randomly divided into a blank group, a model group, a positive control group (metformin hydrochloride, 0.05 g·kg<sup>-1</sup>d<sup>-1</sup>), and an HLWDT (7.8 g·kg<sup>-1</sup>d<sup>-1</sup>) group based on the body weight of rats. The blank group and the model group were fed with the same volume of distilled water. The dose for each group was set as 10 mL·kg<sup>-1</sup>d<sup>-1</sup>. After four weeks of continuous gavage, blood was collected and serum was separated. The skeletal muscles of rats were stored in liquid nitrogen. Subsequently, serum IL-1<italic>β</italic> and IL-18 were detected by the enzyme-linked immunosorbent assay (ELISA). The mRNA and protein expression levels of NLRP3, Caspase-1, and GSDMD were detected by real-time quantitative PCR (Real-time PCR) and Western blot, respectively. The expression of GSDMD, IL-1<italic>β</italic>, and IL-18 proteins in skeletal muscle tissues was detected by immunofluorescence. Hematoxylin-eosin (HE) staining was used to observe the pathological changes of skeletal muscles. Result:Compared with the blank group, the model group showed increased IL-1<italic>β</italic> and IL-18 in serum, and NLRP3, Caspase-1, and GSDMD gene and protein expression in skeletal muscle tissues (<italic>P</italic><0.01). Immunofluorescence assay showed that GSDMD, IL-18, and IL-1<italic>β </italic>protein expression in skeletal muscle tissues of the model group was significantly elevated (<italic>P</italic><0.01). HE staining showed obvious pathological changes in skeletal muscles. Compared with the model group, the HLWDT group and the positive control group could decrease IL-1<italic>β</italic> and IL-18 in serum and NLRP3, Caspase-1, and GSDMD gene and protein expression in skeletal muscle tissues (<italic>P</italic><0.01). In addition, immunofluorescence assay revealed that HLWDT could reduce protein expression levels of GSDMD, IL-1<italic>β</italic>, and IL-18 in skeletal muscles of IGT rats (<italic>P</italic><0.01). The results of HE staining showed that HLWDT could improve the pathological changes of skeletal muscles in IGT rats<bold>.</bold> Conclusion:HLWDT can inhibit skeletal muscle pyroptosis of IGT rats, and the mechanism may be closely related to NLRP3/Caspase-1/GSDMD/IL-18/IL-1<italic>β</italic> signaling pathway.

ABSTRACT

Objective:To observe the effects of Huazhuo Jiedu Shugan Prescription (HZJDSG) on learning, memory, and the expression of phosphoinositide 3-kinase (PI3K)/protein kinase B (Akt)/glycogen synthase kinase-3<italic>β</italic> (GSK-3<italic>β</italic>) pathway-related proteins in epileptic rats, and to explore its possible mechanism. Method:Forty-eight SPF male SD rats were randomly divided into a normal group, a model group, a sodium valproate (0.19 g·kg<sup>-1</sup>) group, and low- (2.7 g·kg<sup>-1</sup>), medium- (5.4 g·kg<sup>-1</sup>), and high-dose (10.8 g·kg<sup>-1</sup>) HZJDSG groups, with eight rats in each group. The normal group received 0.9% sodium chloride solution (0.035 g·kg<sup>-1</sup>) by intraperitoneal injection, and the other five groups received pentetrazol (PTZ) at the same dose to induce a chronic epilepsy model for a total of 14 times. The drug groups received corresponding drugs and the normal group and the model group received 0.9% sodium chloride solution at the same volume once a day for 28 days. During the drug intervention period, epilepsy was maintained in each modeling group by intraperitoneal injection of PTZ on day 7, 14, 21, and 28. The behavioral changes of rats were observed by Morris water maze and the pathomorphological changes of rat hippocampal neurons by hematoxylin-eosin (HE) staining. The protein expression of phosphorylation Akt(p-Akt)and p-GSK-3<italic>β</italic> was detected by immunohistochemistry and the protein expression of PI3K, Akt, p-Akt, GSK-3<italic>β</italic>, and p-GSK-3<italic>β</italic> by Western blot. Result:Compared with the normal group, the model group showed prolonged platform finding time (<italic>P</italic><0.01), reduced number of platform crossings (<italic>P</italic><0.01), structural damage of neurons in the CA1 region of the hippocampus, down-regulated protein expression of p-Akt and p-GSK-3<italic>β </italic>in the CA1 region of the hippocampus (<italic>P</italic><0.05), and reduced relative expression of PI3K, p-Akt, and p-GSK-3<italic>β</italic> in the hippocampus (<italic>P</italic><0.01). Compared with the model group, the sodium valproate group and the HZJDSG groups showed shortened platform finding time (<italic>P</italic><0.01) and improved neuronal structure in the CA1 region of the hippocampus, while the sodium valproate group and the high- and medium-dose HZJDSG groups exhibited increased number of platform crossings (<italic>P</italic><0.01), up-regulated protein expression of p-Akt and p-GSK-3<italic>β</italic> in the CA1 region of the hippocampus (<italic>P</italic><0.05), and elevated relative expression of PI3K, p-Akt, and p-GSK-3<italic>β</italic> (<italic>P</italic><0.01). Conclusion:HZJDSG can improve the learning and memory of epileptic rats, and its antiepileptic effect may be achieved by the activation of PI3K/Akt/GSK-3<italic>β</italic> pathway-related proteins.

ABSTRACT

This study investigated the mechanism of improving impaired glucose tolerance(IGT) of rats by Huanglian Wendan Decoction from the perspective of the skeletal muscle Nod-like receptor protein 3(NLRP3)/cysteinyl aspartate specific proteinase-1(caspase-1)/interleukin-1β(IL-1β), interleukin-18(IL-18) pathway. Healthy male SD rats were fed with the diet containing 45% fat for 20 weeks, accompanied by a high-temperature and high-humidity environment and an inactive lifestyle, for the establishment of the IGT rat model. The rats were divided into the blank control group, model control group, metformin hydrochloride group(positive drug group, 0.05 g·kg~(-1)·d~(-1)) and Huanglian Wendan Decoction group(7.8 g·kg~(-1)·d~(-1)). After continuous intragastric administration for 4 weeks, the obesity and glycemic indexes of all the rats were measured. The fasting serum insulin(FINS) level was determined by ELISA, with the insulin sensitivity index(ISI) and insulin resistance index(IRI) calculated. The mRNA and protein expression le-vels of nuclear factor kappaB(NF-κB), NLRP3, caspase-1, IL-1β and IL-18 in skeletal muscle tissue were detected by real-time polymerase chain reaction(PCR), Western blot and immunofluorescence. Compared with the blank control group, the model control group witnessed significantly increased mRNA and protein expression of NF-κB, NLRP3, caspase-1, IL-1β and IL-18. As revealed by the comparison with the model control group, Huanglian Wendan Decoction could effectively regulate the obesity status, reduce body weight, correct the abnormal levels of 2-hour plasma glucose(2 hPG), insulin resistance index(IRI), insulin sensitivity index(ISI), and lower the mRNA and protein expression of NF-κB, NLRP3, caspase-1, IL-1β and IL-18 in the skeletal muscle tissue of IGT rats. Combined with previous studies, the above results showed that the occurrence and development of IGT was closely related to inflammatory response and the classic pyroptosis pathway in skeletal muscle, such as NLRP3/caspase-1/IL-1β, IL-18. It is inferred that the mechanism of Huanglian Wendan Decoction was to alleviate insulin resistance(IR) and then reverse the course of IGT lies in the regulation of the abnormal insulin receptor signaling pathway based on the NLRP3 inflammasome pathway.

ABSTRACT

To investigate the mechanism of Huanglian Wendan Decoction in intervening impaired glucose tolerance( IGT) rat insulin resistance( IR) based on the pyroptosis pathway of liver cells. The IGT rat model was established by high-fat diet( 20 weeks) combined with high temperature,humidity environment and inactive lifestyle. The experiment was divided into blank group,model group,metformin hydrochloride group( positive group)( 0. 05 g·kg~(-1)·d~(-1)) and Huanglian Wendan Decoction group( 7. 8 g·kg~(-1)·d~(-1)). After continuous intragastric administration for 4 weeks,the body weight,body length and abdominal circumferences of all the rats were measured,the Lee' s obesity indexes was calculated,and the levels of fasting plasma glucose( FPG) and 2-hour plasma glucose( 2 hPG) in each group were measured. Serum insulin levels( FINS) and tumour necrosis factor-α( TNF-α) levels were detected by ELISA,and insulin sensitivity index( ISI) and insulin resistance index( IRI) values were calculated. The expressions of cysteinyl aspartate specific proteinase-1( caspase-1),gasdermin D( GSDMD),interleukin-1β( IL-1β) and interleukin-18( IL-18) m RNA and proteins in liver tissues were detected by Real-time PCR and Western blot. Immunofluorescence was used to detect the expressions of caspase-1 and GSDMD protein in liver tissues. Huanglian Wendan Decoction could effectively reduce the weight of model rats,improve abdominal obesity,FINS,IRI and ISI indexes and correct 2 hPG levels. Compared with blank group,TNF-α levels in serum and caspase-1,GSDMD,IL-1β and IL-18 expressions in liver tissue of model control group were increased significantly. Huanglian Wendan Decoction could effectively reduce TNF-α level in serum,regulate the expressions of caspase-1,GSDMD,IL-1β and IL-18 genes and proteins in liver tissues of IGT rats. The above results showed that the occurrence and development of " obesity-IR-abnormal glucose metabolism-type 2 diabetes mellitus" was closely related to inflammatory response and the classical pyroptosis pathway mediated by caspase-1/GSDMD. The mechanism of Huanglian Wendan Decoction in improving IR may be correlated with reduction of the level of inflammatory factors and the alleviation of the state of pyroptosis,and thus reverse the course of IGT.

ABSTRACT

Phospholipids are important components of biomembrane and lipoproteins. Phospholipids can be oxidized by free radicals/nonradicals and enzymes to form oxidized phospholipids (OxPLs), which can lead to further generation of oxidation products with different biological activities. Clinical evidence shows that OxPLs are constantly generated and transformed during the pathogenesis of atherosclerosis and accumulated at the lesion sites. OxPLs are highly heterogeneous mixtures that can influence the progress of atherosclerosis through a variety of related receptors or signaling pathways. This review summarizes the process of phospholipid oxidation, the related products, the interaction of OxPLs with endothelial cells, monocytes/macrophages, smooth muscle cells, platelets and lipoproteins involved in the pathological process of atherosclerosis, and the progress of the researches using OxPLs as a target to inhibit atherosclerosis in recent years.

ABSTRACT

Objective: To evaluate the application of island flap and combined flap in one-stage reconstruction of nasal alae defects after external nasal tumor resection. Methods: Data of 11 patients with perforating or full-thickness defects of the alae after nasal tumor resection in XuanWu Hospital, Capital Medical University between June 2016 and February 2021 were analyzed retrospectively. There were 7 males and 4 females, and the ages ranged from 51 to 89 years. Island flap, island flap combined with nasolabial flap or V-Y advancement flap, and island flap combined with bilobed flap were applied according to the range of defects. Descriptive statistical method was applied to analyze the treatment effects. Results: All flaps of the 11 patients were successful survival and the incisions were primary healing. Being followed up for 5 to 59 months, the patients had satisfying appearance and ventilation function, and no tumor relapsed. Conclusion: For the patients with nasal alae defects after external nose tumor resection, selecting suitable island skin flap or combined skin flap can be used to reconstruct the ideal nasal appearance and function of the nose.

Rehabilitation is increasingly valued in the field of Parkinson's disease (PD) therapy. European countries issued European Physiotherapy Guideline for Parkinson's Disease in 2014. Based on the analysis of the evaluation method in the guideline, combined with the recent literature and clinical feasibility, this paper mainly analyzed the evaluation tools recommended by the European guidelines for physical therapy of PD, the general requirements for using the recommended evaluation tools and the significance of evaluation, in terms of pre-evaluation tools, evaluation tools to assess the effectiveness of physical therapy, evaluation tools to assess the severity and progression of PD and the evaluation of cognitive psychology.

ABSTRACT

Objective: To study the long-term efficacy of cytoreduction surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) for peritoneal carcinomatosis from colorectal cancer. Methods: The clinical data were collected of 103 patients with peritoneal carcinomatosis (PC) from colorectal cancer undergoing CRS with HIPEC in the Department of General Surgery of the First Medical Center of Chinese PLA General Hospital from January 1, 2012 to May 31, 2013, and divided into colonic group (n=60) and rectal group (n=43) according to the origin position of peritoneal cancer. The surgical status of patients (whether combined with other organ resection, number of stoma, peritoneal carcinomatosis index (PCI), completeness of cytoreduction (CC) score, transfusion, etc.) and the short-term complications and long-term survival after operation in the both groups were compared. Kaplan-meier method was used to draw the survival curve of patients, and log-rank method was employed to compare the survival differences between the two groups. Results: There was no significant difference in the surgical data between the two groups (PCI, CC score, hepatectomy, number of anastomosed stoma, transfusion, etc., P>0.05). The follow-up time of the 103 patients was (28.43±15.65) months, median survival time (OS) was 26.3 months, median disease free survival time (DFS) was 17.7 months, median recurrence to time of death (TTD) was 18.3 months. The 3-year and 5-year survival rate were 35.9% and 8.7%, respectively. The patients' survival was better in colonic group than in rectal group (OS: 28.5 months vs. 19.7 months; DFS: 21.1 months vs. 13.6 months; TTD: 22.3 months vs. 12.1months; 3-year survival rate: 48.3% vs. 18.6%; 5-year survival rate: 15.0% vs. 0, P<0.05). Conclusion: When CRS with HIPEC is implemented, the incidence of short-term complications in patients with PC originated from colon may match to those with PC originated from rectum, while the long-term efficacy is better in the former than in the latter.

OBJECTIVE@#To evaluate the efficacy and safety of topical delivery of modified Da-Cheng- Qi Decoction (, MDCQD) by low-frequency ultrasound sonophoresis (LFUS) in patients with refractory metastatic malignant bowel obstruction (MBO) using an objective performance criteria (OPC) design.@*METHODS@#Fifty patients with refractory metastatic MBO were enrolled in this open-label single-arm clinical trial. Alongside fasting, gastrointestinal decompression, glycerol enema, intravenous nutrition and antisecretory therapy, a 50 g dose of MDCQD (prepared as a hydrogel) was applied through topical delivery at the site of abodminal pain or Tianshu (S 25) using LFUS for 30 min, twice daily for 5 consecutive days. The overall outcome was the remission of intestinal obstruction, and improvement on abdominal pain, abdominal distention, nausea and vomiting scores. Indicators of safety evaluation included liver and renal function as well as blood coagulation indicators.@*RESULTS@#Among 50 patients, 5 patients (10%) showed complete remission of intestinal obstruction and 21 patients (42%) showed improvement of intestinal obstruction. The overall remission rate of bowel obstruction was 52%. The results of the symptom score, based on the severity and frequency of the episode, are as follows: 26 patients (52%) showed improvment on symptom scores, 20 patients (40%) did not respond to treatment, and 4 patients (8%) discontinued treatment due to intolerance. No serious adverse effects or abnormal changes on liver and renal function or blood coagulation were observed.@*CONCLUSION@#Topical delivery of MDCQD at 100 g/day using LFUS can improve the treatment response in patients with refractory metastatic MBO.
